Mostly Harmless

... was described on the cover of its first edition as "The fifth book in the increasingly inaccurately named Hitchhikers Trilogy."

At the start of the series, Arthur Dent discovers that the entry for Earth in the Hitchhiker's Guide consists, in its entirety, of the word "Harmless". Ford Prefect (a contributor to the Guide) explains to Arthur that he has spent the last 15 years researching an article on Earth, and assures him that it will be included in full in the next edition. But after editorial cuts due to space restrictions, the revised entry simply reads "Mostly harmless."
